Killer Queen is a wild beauty with light brown eyes and long blonde hair that reaches past her waist, the section at the end fastened by an ornament consisting of several star-shaped objects. She has large breasts and a voluptuous body, but because of her fierce personality, the only people who have an interest in her are masochists. She typically wears a long-sleeved blue dress with gold accents, the precise design of which varies between adaptations. It is cut short like a skirt to reveal her white undershorts and thigh-high stockings, while her black leather boots are adorned with golden armor. She also wears a simple choker around her neck which resembles a metal ring in the light novels. In the anime it is black in color, and adorned with a small silver medallion.
