Raoh (ラオウ, Raō), also known by his alias Ken-Oh (拳王, The King of Fists), is the overall main antagonist of Hokuto no Ken. Raoh is one of the inheritors and practitioners of Hokuto Shinken. Unlike his brother Toki, who used his fist to heal the sick, or Kenshiro, who used his fist to protect the innocent, Raoh chose to become the most powerful martial artist of all time, and used his fist to become a mighty conqueror of the wasteland, subduing all people in his wake.

Souther (サウザー, Sauzā) is the main antagonist of the Souther Arc in Hokuto no Ken. He is also responsible for causing chaos among the Nanto Rokusei Ken by manipulating Yuda to join Raoh's empire. He is the self proclaimed Holy Emperor whom had kidnapped children as labor for his mausoleum.

A fellow practitioner of Hokuto Shinken who trained together with Kenshiro. Jealous of Kenshiro being named the successor, his resentment of his younger brother’s talent soon festered into hatred. He wears a bizarre helmet to hide his face, and his chest is adorned with the same scars as Kenshiro’s. He has the crazed eyes of a mad dog, and a dangerous, unstable mind to match.

Ryuga (リュウガ, Ryūga) is the main antagonist during the Ryuga arc and lieutenant of Raoh's empire. He helped consolidated Raoh empire during his master's recovery from his fight with Kenshiro. He sought to fight Kenshiro to test which of the Hokuto brothers would restore order to the wasteland.
